With nipple piercings it depends on what sort of nipples you have. Everybody's body is different as we all know.
I have both nipples pierced and they don't stick out at all.

It depends how sensitive they are.
My girlfriend had it done and she said it felt fine. But her friend was in complete agony and got rid of it after a month or two. There are ways to
desensitize them.

Like someone else said, it depends on the person. For me tha pain was moderate. I wouldn't say it was awful but it was still fairly intense
pain.
I know a girl who has her clitoral hood pierced and she claims it did not hurt her one bit to get it done, which is not what I would have expected
given that it's probably the most sensitive part of the female body.
